AClinical Manager Specialist:

  • Coordinate and oversee all direct and indirect patient/client services provided by clinical staff.
  • Supervise Patient Care Secretary.
  • Define and communicate expectations regarding quality provision of services and productivity to interdisciplinary team to assist them in continually improving all aspects of Company services.
  • Provide regular communication with the hospice staff, including clinical staff, regarding pertinent organizational and clinical issues.
  • Facilitate effective interpersonal communication between staff members and promote effective administrative function throughout the organization.
  • Responsible for the oversight of the assessment and evaluation, intervention and documentation of patient and family needs.
  • Ensure that all Conditions of Participation and Standards of Care are being met or exceeded.
  • Manage the CQI program as directed. This would include conducting a quarterly Designated IDT/CQI and safety committee meeting, coordinating with committee members on assigned surveys and audits, ensuring the completion of all required audit results and reports (per CQI Plan), and completing meeting minutes and maintain the CQI Manual.
  • Ensure Interdisciplinary Team (IDT) staffing meets requirements of Company ratios per census.
  • Interview and hire IDT members.
  • Complete and conduct annual performance evaluations in a timely manner and complete any corrective action plans through the HR Department.
  • Assure requirements and standards are maintained by all IDT members including productivity standards.
  • Oversee maintenance of patient/client clinical records, statistics, reports and records for purposes of evaluation and reporting of agency activities ensuring all patient medical record documentation is present per Company HealthCare Policy and Procedures and in compliance with local, state and federal law.
  • Oversee and approve IDT schedules.
  • Responsible for management of Pharmacy costs within budget.
  • Responsible for management of DME, labor, mileage, and suppl costs within budget.
  • Implement and ensure orientation for new personnel.
  • Assure compliance with local, state and federal laws, Medicare regulations, HIPAA and established Company HealthCare policies and procedures.
  • Maintain Company Quality Improvement Program in conjunction with CQI Manager.
  • Participate as indicated in monthly Patient Care Department Report.
  • Plan and implement in-service and continuing education programs to meet education and training needs of personnel as required.
  • Participate in staff meetings, department meetings, team meetings, in-services, committees and other related activities as needed.
  • Participate in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.
  • Serve as Administrator On-Call as required.



Job Requirements

Qualifications
  • Registered Nurse (RN) licensed or in accordance with the Board of Nurse Examiners for Nurse Licensure Compact (NLC).
  • Bachelors degree and A.C.R.N.H. certification preferred.
  • Minimum of one year experience as an RN obtained within the last 36 months.
  • Minimum of two years clinical management experience, including the supervision of nursing staff preferred.
  • Minimum of three years Hospice, home health or related organization experience preferred with understanding of Hospice care provided through an interdisciplinary team.
