Overview: Position serves as a social worker msw.Masters prepared social worker. Plans, organizes and implements social work services for patients and families. Provides counseling; assessment, support and crisis intervention for patients/families/significant others regarding psychosocial adjustment to illness. Develops and implements appropriate treatment plans. Provides information referral and consultation and assists in the discharge planning process. Special Skills:Knowledge of clinical social work practices and regulation standards within a healthcare setting. The knowledge base of psycho-social assessment and intervention, continuum of care and case management methods and standards, excellent communication and documentation skills. Demonstrates knowledge of Age-Specific Criteria, National Association of Social Work (NASW) Standards of Practice. Minimum Requirements: State of Arizona licensed social worker (LMSW). MIDAS and Curaspan to be successfully completed with first 90 days of hire. Position requires masters prepared, licensed social worker or masters prepared licensed professional counselor. Minimum two years experience in medical social work. Three years experience in an acute care setting. Preferred Qualifications: LCSW, Accrediced Case Manager (ACM)
